PARADE 360: Transforming Medication Safety Through Data, Partnerships, and Community Care

10/14/2026

Medication-related harm remains one of the leading causes of preventable emergency department visits, hospitalizations, and poor health outcomes among older adults and individuals with complex medical needs. Many patients receive care from multiple providers, take numerous medications, and face social and economic barriers that can make medication management especially challenging. These factors create opportunities for medication errors, adverse drug events, and gaps in care that too often go undetected until a serious health crisis occurs.

PARADE 360 is an innovative initiative designed to address these challenges through a proactive, community-centered approach to medication safety. Led by PCCI in partnership with Parkland Health, Dallas County Health and Human Services, and community pharmacy partners, the program combines predictive analytics, clinical expertise, and community engagement to identify patients at greatest risk for medication-related harm and connect them with timely, personalized support.

Using advanced risk prediction tools, PARADE 360 helps care teams focus resources on the patients who need them most. Pharmacists, community health workers, and community partners work together to identify potential medication issues, provide patient education, coordinate care, and address barriers that may contribute to unsafe medication use. By extending support beyond traditional clinical settings and into the community, the program aims to improve medication management, strengthen care coordination, and reduce preventable adverse events.

The initiative also seeks to demonstrate how data-driven insights can be transformed into meaningful action through strong cross-sector collaboration. By bringing together healthcare providers, public health agencies, and community pharmacies, PARADE 360 creates a connected ecosystem focused on protecting vulnerable patients and improving outcomes.

Over the course of the project, the team will evaluate the impact of the model on medication safety and patient outcomes while developing implementation resources that can help other organizations adopt and replicate the approach. Ultimately, PARADE 360 aims to establish a sustainable, scalable framework for medication safety that can be expanded to additional populations and communities.

PARADE 360 reflects PCCI's commitment to leveraging data, technology, and community partnerships to advance safer, more equitable care and improve the health and well-being of the populations we serve.
